The eyes sit still with no beating at rest, in gaze, or after a positional maneuver. A negative exam is still informative: it rules BPPV in or out on repeat testing, and a truly silent exam in a patient who is symptomatic right now should prompt a closer look at the other two HINTS signs rather than a reflexive 'normal'.
